Vaccine Exemptions in Georgia

The state of Georgia has a religious exemption for vaccination for all students attending public schools and childcare facilities. Parents are required to fill out the Affidavit of Religious Objection to Immunization, Form 2208 and then file the form with the school or childcare facility.
